How to apply for




1. Apply for Scheduled Tribe/ Caste Certificate

Application form for Scheduled Tribe/ Scheduled Caste Certificate is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Two Passport Size Photographs
  2. Birth Certificate/SSLC Admit Card
  3. Any one of Residential Proof (EPIC/ Ration Card/patta/Electricity Bill/ Telephone Bills)
  4. Parent’s SC/ST Certificate
  5. If adopted, Adoption Documents (Court Order/ Registered Adoption Deed)

2. Apply for Permanent Residential Certificate

Application form for Permanent Residential Certificate is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Two Passport Size Photographs
  2. Birth Certificate/School Certificate
  3. Any one of Residential Proof ( EPIC /Ration Card/patta /Electricity Bill / Telephone Bills)
  4. Certificates of all institutions in Meghalaya where Studied in
  5. Supporting Documents why Permanent Residential Certificate is required

3. Apply for Recruitment in Armed /Para Military/ PoliceForces

Application form for Armed Para Military Certificate is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Two Passport Size Photographs
  2. Birth Certificate/School Certificate
  3. Any one of Residential Proof (EPIC/Ration Card / patta/ Electricity Bill / Telephone Bills )
  4. Certificates of all institutions in Meghalaya where Studied in
  5. Proof of migration (if applicable)
  6. Supporting Documents why Armed/Para Military Certificate is required

4. Apply for Residential Certificate

Application form for Residential Certificate is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Two Passport Size Photographs
  2. Birth Certificate/School Certificate
  3. Any one of Residential Proof (EPIC/Ration Card / patta/ Electricity Bill / Telephone Bills )

5. Apply for Senior Citizen Certificate

Application form for Senior Citizen Certificate is available at PFC, DC’s Office. Only citizens attaining the age of 60 can apply. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Two Passport Size Photographs
  2. Birth Certificate / Age Certificate from Government Doctor
  3. Pension Payment Order (for retired government servants)
  4. Any one of Residential Proof (EPIC/ Ration Card / patta / Electricity Bill / Telephone Bills)
  5. Blood Group

6. Apply for Income Certificate

Application form for Income Certificate is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Pension Payment Order/Certificate from Treasury Officer(for retired government servants)
  2. Proof of Salary in form of Affidavit (for non-Government applicants)
  3. Any one of Residential Proof (EPIC/Ration Card / patta / Certificate from Area Development Council / Electricity Bill/ Telephone Bills)
  4. Salary Certificate from Employer in case parent(s)/Applicant are employed
  5. Proof of requirement for the Certificate, if applicable
  6. Declaration if either of them is not employed

7. Apply for Registration of Society

Application form to register a society or Association etc is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Copies of the following documents are to be submitted along with the application form duly filled in:-

  1. Constitution of the Society/ Association
  2. Treasury Challan
  3. Seal of the Office Bearer, Round Seal


8. Apply for Registration of Marriage

Application form is available at the Registration Branch, DC Office. Duly filled in form is to be submitted with the following supporting documents as per the Guidelines and Rules & Regulations for Registration under the Special Marriage Act 1954

  1. The application for the Registration should be submitted 30 days before the intended marriage date for the couple (married/unmarried).
  2. During the time of marriage minimum of 3 witnesses should be present.
  1. Birth Certificate should be submitted for the age proof
  2. Residential proof of the individuals.
  3. Marital Status, in case of married couple (marriage certificate, wedding pictures etc). In case of unmarried couple (Marital status certificate).
  4. For Foreigner, i. Visa ii. Passport. iii. Spinster/Bachelor Certificate for Unmarried.
  5. For Divorcee, Divorce decree.
  6. 2 numbers of recent passport size photos.


9. Apply for Permission for Special Event

Application form permission to hold special events is available at PFC, DC’s Office. Form can also be downloaded from the district web site. Attested copies of the following documents are to be submitted along with the application form duly filled in:-

  1. NOC from the DMHO for (Mela & Trade Fair)
  2. NOC from PWD(Building) for (Mela & Trade Fair)
  3. NOC from MeECL for (Mela & Trade Fair)
  4. NOC from SP Fire Service for (Mela & Trade Fair

10. Apply for Registration of Land

A Deed agreement between the Seller & the Purchaser is prepared in a non Judicial Stamp paper, where the amount of Stamp Duty is calculated based upon the prevailing rate of the Land. A complete details of the Land along with the transactional value & Land Schedule is mentioned in the Deed.
The following Documents are to be submitted:-

  1. All documents related to the land to be registered, including maps
  2. ID proof of individuals.
